Chasing health provides follow ups for patients who are discharged with uncontrolled hypertension from the emergency room. We offer nutrition counseling, follow up with home BP measurements and evaluate their medication for better BP control to reduce readmission rate to the hospital.
CDPHP
Chasing Health collaborates with CDPHP to provider high blood pressure screenings, assess for social determinants of health, and offers health literacy education to people in underserved communities in the Capital Region.
Healthy Alliance
Chasing Health renders nutrition counseling, health literacy education for hypertension and diabetes through Healthy Alliance referral platform.
Refugee and Immigrant Support Services of Emmaus (RISSE)
Chasing Health collaborates with RISSE to provide blood pressure screenings, health literacy education for hypertension, diabetes and screen for social determinants of health in underserved local communities in Albany, Troy and Schenectady.
YWCA of Northeastern NY Homeless Shelter
Chasing Health provides once a month FREE clinic to the women’s shelter. Clients comes with multiple medical concerns and questions. Services includes BP screenings, treatment of acute and chronic diseases in collaboration with Chasing Health Integrative Care Nurse Practitioner.
Capital City Rescue Mission Schenectady- New Life for Men
Chasing Health provides monthly Free blood pressure screenings, and nutritional counseling and makes referral to their primary care clinic as needed.
